Injury form PowerApps is designed and developed to help the employees and the managers submit their data if there is any mishap took place during the working hours or else wise and the organisation is taking care of the insurance, It has a 7 step process which allows the entire details of the employees being handled on a single PowerApps so that the organisation can collect everything at once and the employee doesn’t needs to worry about going from place to place to fill out multiple forms.

Start with a 0-60 form

  • Enter all of your basic information on this screen to be submitted as the initial detail form
  • The application comes in 2 languages, English and Spanish, you can choose whatever you’re comfortable with
  • There is a section to attach you signature to verify your authenticity, this option is only to verify if you are refusing the care provided by the company

Injury form listing

  • This is a space where the user will be able to see the listing of all the injuries being added by the users
  • The admin/managers can switch between the companies, select from the date range, choose the location or filter by the status of the forms being submitted
  • While filtering, the admin/manager also gets an option to export these listing as an excel report

Injury form

  • This is the heart of the application, where it has the entire flow starting from reviewing the form entered by the user to approve that particular injury
  • Roles and permissions are managed for the admin to allow the users/executives to access particular tabs so that they only collects the information which they are allowed to
  • Options to lock the injury form once the information is filled up so that it cannot be edited by any user, manager but just the admin
